Not me personally but I have seen this mentioned by others a couple of times. It seems the consistent theme is using mild 115 grain ammo. I noticed on my EVO the slide is fairly hard to rack, I suspect the average 115 grain target loads are just not cycling the slide quite enough to get lock back.
Just a hypothesis at this point, but it would be worth your while to try some warmer 124 or 147 grain loads.
Mine works perfectly but I have not yet tried any 115 grain loads, had zero issues with 124s and 147s though.
